1.        Secure and submit 4 copies (3 copies if from hospital) of fully accomplished Death Certificates within 24 hours

2.        Autopsy is necessary for Medico Legal cases such as untimely death, accident, DOA, 65 and below, or as requested by the family

3.        Confirm intended place of internment for processing of burial permit and scheduling of funeral services and arrange with your memorial park agent the burial date

4.        Only the Certified True Copy of the Death Certificate is needed for all official transactions.  It must bear the seal/stamp and civil registry number of the municipality where the person passed away

5.        For Memorial Plan Holders:
a.   All requests should be addressed to your plan’s servicing
      department or service representative

b.   Please request acknowledgement receipts from your
     plan’s office.  The official receipts MMCI issues are
      only for clients who avail of other services like upgrade,
      chapel use extension, conveyance, multi-viewing, etc

6.        We extend family assistance to look for priest,    police escort, family car, service shuttle, memorial lot, columbarium, and facilitate cremation.  If available, these outside service entail extra charges

7.        Your attending service manager may assist you with obituary notices in the Phil Star, Phil Daily Inquirer, and Manila Bulletin
